Integrated Health Information Exchange

Wherever they were
treated, it’s in the chart.

From regional HIEs to ACO partners, hospital systems, IPAs, ASCs, and your patients’ own iPhones, Althea Smart EHR connects natively to the networks your patients move through. Pull a hospital discharge from Providence or Steward, gap-in-care alerts from Aledade, eligibility from Regal, an HIE summary from Contexture, an op report from Mednet, and home vitals from Apple Health, without leaving the chart.

WHY PRACTICES CHOOSE ALTHEA INTEGRATIONS

The end of blind encounters.

Stop walking into rooms with half the picture. Stop chasing outside records by fax. Stop ordering tests the patient already had last week. Althea Smart EHR plugs directly into the regional HIEs, ACOs, hospital systems, IPAs, and ASCs your patients actually use, so the chart is whole no matter where care happened.

Every Network You Need

Direct, bi-directional integrations with regional HIEs (Contexture), ACO platforms (Aledade, Steward), IPAs (Regal), hospital systems (Providence), RCM platforms (Rivet), ASC networks (Mednet), and patient-generated data from Apple Health, with more added on request.

Import Directly into Chart

Review external data with ease, selecting only the information you need to import into the patient’s chart. No mass-merging duplicates, no junk records, no rebuilding the medication list by hand.

On-Demand & Real-Time

Eligibility checks, ADT notifications, gap-in-care alerts, and document queries run on demand or in the background. Walk-ins, transfers, and ED arrivals surface automatically, no waiting on a fax.

HOW IT WORKS

From network event to chart-side insight.
In four steps.

No matter which network the data originates in, an HIE, an ACO, an IPA, a hospital system, or a surgery center, the path from external event to clinical action inside Althea Smart EHR is the same.

1
Connect

Bi-directional interfaces are stood up with each network you participate in, HIE, ACO, IPA, hospital, ASC, RCM, using HL7, FHIR, or IHE-XCA.

2
Receive

ADT notifications, gap alerts, eligibility, lab results, discharge summaries, and op reports flow into Althea continuously, partner-tagged at the source.

3
Surface

Inbound items appear at the patient level with a clear source tag (Providence, Aledade, Contexture, Apple Health, etc.), so providers know exactly where each record came from.

4
Review & Import

Provider reviews and selectively imports relevant data into the chart, audit-trailed end to end. Nothing imports without an explicit choice.

A week in the life of one patient. Three different networks generate events.
Every one of them lands, source-tagged, in the same Althea Smart EHR chart.

Patient, J. Morales · 47F · T2DM, HTN
Across the week: 1 ED visit · 1 cardiology Rx · 1 lab panel · daily home vitals
4 Networks, 1 Chart
Inbound from Networks
Mon, Apr 21 · Providence ED discharge summary received via PSJH Clinical Data Exchange. St. Joseph ED visit for chest pain, R/O ACS, troponin negative, discharged home.
Tue, Apr 22 · Contexture HIE New cardiology e-prescription appears in HIE feed: Atorvastatin 40mg, prescribed by Dr. Patel at an outside cardiology practice.
Thu, Apr 24 · Aledade Gap-in-care alert fires: A1C overdue, last value 7.4% three months ago. Recommended action populated for the next office visit.
Fri, Apr 25 · Contexture HIE Outside lab results posted: A1C 8.1 (H), LDL 138 (H). Drawn at independent draw site Apr 18.
Sat-Sun, Apr 26-27 · Apple Health Home BP readings shared from patient’s iPhone (consented): 4-day average 142/89, resting HR 78. Flagged for hypertension follow-up.
Mon, Apr 28 Encounter, Source-Tagged
PROV ED visit summary tagged Providence. Provider reviews, imports, and discusses the chest pain workup with the patient.
HIE New atorvastatin Rx tagged Contexture. One-tap import to in-house med list, no manual reconciliation.
ACO Gap-in-care alert tagged Aledade. A1C and lipid trend already populated, supporting the visit’s decision-making.
PGHD Home BP and HR tagged Apple Health. The 142/89 average informs the hypertension conversation, no recall, no guesswork.
PLAN Provider walks in with the full picture. No phone calls, no faxes, no “I didn’t know about that visit.”

Frequently Asked Questions

Althea Smart EHR Network Integrations

A dedicated answer for each partner, what the integration is, and what data flows.

Aledade · ACO Platform
What does the Aledade integration do?

Aledade is a value-based-care and ACO platform serving thousands of independent primary-care practices. The integration brings hospital event notifications (ADT), gap-in-care alerts, risk-adjustment suggestions, and ACO quality-measure data directly into the Althea Smart EHR chart and provider worklist. Practices participating in MSSP, Medicare Advantage, Medicaid, or commercial value-based contracts see what they need to act on, in the chart, instead of in a separate Aledade app.

Contexture · Regional HIE
What does the Contexture integration do?

Contexture is the largest HIE in the western U.S., covering Arizona and Colorado with roughly 28 million patient records from hospitals, labs, and providers. The integration pulls ED visits, hospital admit/discharge/transfer (ADT) feeds, lab and imaging results, immunizations, problem lists, medications, and CCD/CCDA documents into Althea Smart EHR. Bi-directional, so encounter summaries from your practice flow back to Contexture, contributing to the regional record.

Providence · Health System
What does the Providence St. Joseph Health integration do?

Providence operates 51 hospitals and hundreds of clinics across Alaska, Washington, Oregon, California, Montana, New Mexico, and Texas. The integration uses PSJH’s Clinical Data Exchange to deliver diagnoses, lab and imaging results, medications, and discharge summaries from any Providence facility into the patient’s Althea Smart EHR chart, with patient consent honored per Providence’s opt-out policy.

Regal Medical Group · IPA
What does the Regal Medical Group integration do?

Regal is part of Heritage Provider Network, one of Southern California’s largest physician-led IPAs, with 500,000+ members across six counties. The integration handles real-time eligibility verification, member roster updates, prior-authorization status, capitated claims data, and referral/authorization tracking, so a Regal-attributed patient’s coverage and authorizations are visible in the chart without logging into Regal Express Access separately.

Rivet Health · RCM & Pricing
What does the Rivet Health integration do?

Rivet (now part of Zelis) is a revenue-cycle and pricing-transparency platform. The integration brings real-time eligibility and benefits data, payer contracted rates, automated patient cost estimates (No Surprises Act compliant), and payment-variance / underpayment detection into Althea Smart EHR. Front-desk staff present accurate estimates at check-in; back-office staff catch underpaid claims before they age.

Steward · Health System & ACO
What does the Steward Health Care Network integration do?

Steward Health Care Network is a national hospital and ACO organization. The integration provides ADT notifications from Steward facilities, ACO performance and quality-measure data, member attribution rosters, and managed-care contract performance reporting. Practices in Steward-affiliated networks see hospital events in real time and track value-based-contract performance from inside the EHR.

Mednet · ASC Platform
What does the Mednet integration do?

Mednet’s OASIS+ platform powers ambulatory surgery centers across the country. The integration closes the loop between primary-care referrals and surgical care, delivering scheduled procedure dates, intra-op records, post-op notes, and operative reports from the ASC back into the referring provider’s Althea Smart EHR chart. The PCP knows the surgery happened, what was done, and any post-op instructions, without chasing a fax.

Apple Health · Patient-Generated Data
What does the Apple Health integration do?

Apple Health is the device-and-app health hub on iPhone, iPad, and Apple Watch. Built on the SMART on FHIR standard, the integration lets patients consent to sharing their own data into the chart, including home blood pressure readings, heart rate, ECG, blood oxygen, glucose readings, weight, activity and exercise minutes, sleep, and irregular-rhythm notifications. Useful for hypertension follow-up, AFib monitoring, diabetes management, and any condition where what happens between visits matters as much as what happens at the visit.

Compliance
Are these integrations HIPAA-compliant?

Yes. All traffic moves over encrypted, audit-logged channels (TLS 1.2+) under signed data-sharing agreements between MD Synergy and each partner. Patient consent is captured per the practice’s policy and the partner’s opt-out terms. Every inbound record, query, and import is audit-trailed with user, timestamp, and patient context.

New Partners
What if my preferred network isn’t on the list yet?

MD Synergy actively onboards additional HIEs, ACOs, IPAs, hospital systems, and ASCs as practices need them. If your preferred partner supports HL7, FHIR, IHE-XCA, or a comparable interoperability standard, an interface is typically achievable. Reach out and the integrations team will scope it for your practice.
