The integration brings hospital event notifications (ADT), gap-in-care alerts, risk-adjustment suggestions, and ACO quality-measure data directly into the Althea Smart EHR chart and provider worklist. Practices participating in MSSP, Medicare Advantage, Medicaid, or commercial value-based contracts see what they need to act on, in the chart, instead of in a separate Aledade app.
Contexture is the largest HIE in the western U.S., covering Arizona and Colorado with roughly 28 million patient records from hospitals, labs, and providers. The integration pulls ED visits, hospital admit/discharge/transfer (ADT) feeds, lab and imaging results, immunizations, problem lists, medications, and CCD/CCDA documents into Althea Smart EHR. Bi-directional, so encounter summaries from your practice flow back to Contexture, contributing to the regional record.
Providence operates 51 hospitals and hundreds of clinics across Alaska, Washington, Oregon, California, Montana, New Mexico, and Texas. Steward Health Care Network is a national hospital and ACO organization. The integration provides ADT notifications from Steward facilities, ACO performance and quality-measure data, member attribution rosters, and managed-care contract performance reporting. Practices in Steward-affiliated networks see hospital events in real time and track value-based-contract performance from inside the EHR.
Mednet’s OASIS+ platform powers ambulatory surgery centers across the country. The integration closes the loop between primary-care referrals and surgical care, delivering scheduled procedure dates, intra-op records, post-op notes, and operative reports from the ASC back into the referring provider’s Althea Smart EHR chart. The PCP knows the surgery happened, what was done, and any post-op instructions, without chasing a fax.
Apple Health is the device-and-app health hub on iPhone, iPad, and Apple Watch. Built on the SMART on FHIR standard, the integration lets patients consent to sharing their own data into the chart, including home blood pressure readings, heart rate, ECG, blood oxygen, glucose readings, weight, activity and exercise minutes, sleep, and irregular-rhythm notifications. Useful for hypertension follow-up, AFib monitoring, diabetes management, and any condition where what happens between visits matters as much as what happens at the visit.
Yes. All traffic moves over encrypted, audit-logged channels (TLS 1.2+) under signed data-sharing agreements between MD Synergy and each partner. Patient consent is captured per the practice’s policy and the partner’s opt-out terms. Every inbound record, query, and import is audit-trailed with user, timestamp, and patient context.
